The Long Bed Request form for the 2012-13 academic year will be available on MyChecklist from June 1, 2012 – June 30, 2012.

• You MUST use your uark.edu address when requesting a long bed
  • All requests from a non-uark.edu address will be rejected
• Filling out a request does not guarantee you will receive a long bed
  • We have a limited supply of long beds
• You will be notified via your UARK email AFTER July 15, 2012 regarding the status of your request
  • We are unable to give you a status update prior to that date
